ABSTRACT

Purpose: This qualitative study aimed to explore the perspectives of adolescents and young adults (AYA) on a planned nurse-patient dyadic storytelling intervention. Background: Cancer is a highly distressful event for AYA. AYA with cancer experience multidimensional suffering while dealing with their developmental transition from adolescence to young adulthood. Their unique needs require appropriate, well-tailored psychosocial support. Nurses can provide such support through storytelling approaches. METHOD: AYA cancer survivors participated in cross-sectional qualitative interviews to provide feedback on the nurse-patient storytelling intervention model. ANALYSIS: Qualitative content analysis was used to interpret and categorize the data. FINDINGS: Barriers and facilitators of the intervention emerged. Themes related to barriers included (a) the disadvantages of an online program, (b) the limitations of the in-person program, (c) interacting with nurses, (d) sharing personal stories with others, and (e) the timing of the program. Themes regarding facilitators included (a) benefits to AYA with cancer, (b) benefits to nurses, (c) benefits of online/in-person programs, and (d) willingness to participate. Conclusions: AYA with cancer may benefit from the nurse-patient dyadic storytelling intervention. Developing innovative methods to optimize and customize interventions based on preferences is essential. Future research should involve nurse feedback and tailored approaches for AYA with cancer.

ABSTRACT

AIM: The increase in the number of people with chronic obstructive pulmonary disease (COPD) and the disease burden, has prompted concerted efforts to improve healthcare, particularly outpatient services. In line with these attempts the Partnership-Based Nursing Practice Theoretical Framework for People with COPD was developed to guide outpatient nursing care. The principal approach of the framework is a 'Dialogue' with the patients, which has four components: 'Establishing family involvement', 'Assisting living with symptoms' and 'Facilitating access to healthcare', with the primary goal being 'Enhancement of the health experience'. With new knowledge, research on the framework, and extensive experience in using it, a need arose to modify the framework to maximize its clinical utility. DESIGN: Discursive paper. METHODS: A narrative review and critical reflection was conducted to revise the nursing practice framework via selected literature search from 2012 to 2022, research on the framework, and the authors' reflections on the clinical experience of using the framework. RESULTS: The nursing practice framework highlights capacities and possibilities that lie in the nurse-patient relationship. The overarching dialogue in the revised framework includes both patients and families. The action-related component 'Assisting living with the disease' was added to the framework to underscore the significance of attempting to understand what may lie ahead for patients and families. The other action-related components are as follows: 'Assisting living with symptoms' and 'Facilitating access to healthcare'. The primary goal remains unchanged: enhancing the 'Health experience'. CONCLUSION: Using the revised nursing practice framework in outpatient care may help to enhance the lives of people with COPD and their families, particularly at advanced stages of the disease. It may have transferability to other groups of people living with progressive diseases dealing with complicated health problems, and to reduce the usage of costly healthcare resources such as hospital care. IMPLICATIONS FOR THE PROFESSION AND/OR PATIENT CARE: The partnership-based nursing practice framework assumes an extension of conventional specialized respiratory service and embraces a comprehensive account for that which may influence the patient's health problems. This guidance, which holistically attends to patient-family needs of living with complicated and progressive health predicaments, is fundamental. It contributes to strengthening the disciplinary focus of nursing, interdisciplinary collaboration, person-family-centred quality nursing care and inspires research initiatives. Critical reflections and updates on nursing practice frameworks, such as this revision, are essential to advance nursing and healthcare. PATIENT OR PUBLIC CONTRIBUTION: There is no direct patient- or public contribution.

ABSTRACT

AIM: To describe the meaning of caring for older persons based on the experiences of nursing students in Greece. BACKGROUND: The present study is conducted based on a caring science perspective derived from a holistic viewpoint of human beings, aiming to support the well-being of older persons. In Greece, informal caregivers play an important role in caring for older people; however, as the population ages, the need for registered nurses specialising in caring for older people is also increasing. Previous research in other countries has reported a relative lack of interest in this field, which is a threat to the quality of care. To ensure an adequate number of new-generation nurses caring for older people, nursing students' perceptions of the phenomenon of caring for older persons should be well understood and explored in a context where the number of long-term care beds and residential care facilities are low and where informal caregiving is common. DESIGN: Based on Descriptive Phenomenology, in line with the Reflective Lifeworld Research (RLR) approach. METHODS: A total of 12 nursing students at a university in Greece underwent in-depth lifeworld interviews online during June and July 2021 and March 2022; Interviews were transcribed verbatim and were analysed according to the principles of RLR that approaches the phenomena with openness and constant reflection. The participants were students in their fourth or fifth year of nursing education; they had previous experience in older person care and were not acquainted with the authors. RESULTS: The results show the essential structure of the meaning of caring for older persons, where otherness consists of intertwined demanding and rewarding elements. The essence is further conveyed by three constituents: otherness of the older person: witnessing someone's vulnerability; trustful caring: struggling with normative structures; and closeness and distance: balancing between familiarity and professionalism. CONCLUSION: To ensure quality of care for older persons in the future, an understanding of how nursing students describe their experiences of caring for older persons during education is important. Balancing between familiarity and professionalism highlights the need for further reflection on professionalism and dignity and identifying the norms and values helps to highlight particularities of the context and national healthcare system. Curricula focusing on cultivating students' preunderstanding, regarding caring for older persons are required to promote a higher quality of care in the future. Caring science can contribute to a focus on a holistic perspective in caring for older people. TWEETABLE ABSTRACT: There is an ever-increasing need for educated nurses in the field of caring for older people. With the lack of interest in this field of nursing, the overall quality of care is impacted. This study describes the meaning of caring for older persons based on the experiences of nursing students in the Greek health care system.

ABSTRACT

This study aimed to describe perceptions and experiences of nurses and midwives working in primary care about domestic violence (DV) in pregnancy. Data were collected by conducting in-depth face-to-face interviews with 10 midwives and 7 nurses working in seven family health centers between July 17 and August 28, 2020. Five main themes emerged: "The causes of domestic violence," "Difficulty recognizing domestic violence," "Obstacles to revealing domestic violence," "Obstacles to Assisting/Supporting domestic violence in pregnant women," and "Solutions to prevent domestic violence in pregnancy." Midwives/nurses feel inadequate and unprepared to recognize the symptoms of DV and to guide and support women and they are willing to take an active role in managing DV. It is recommended that they should be trained on DV, a monitoring system should be established including primary healthcare services and a measurement tool should be developed.

ABSTRACT

AIMS: To test a model for person-centred pain management using qualitative evidence in the literature and refine it based on the results. DESIGN: A qualitative systematic review with thematic synthesis using the Fundamentals of Care framework. METHODS AND DATA SOURCES: A literature search in February 2021 in six scientific databases: CINAHL, PsycInfo, Pubmed, Scopus, Social Science Premium Collection and Web of Science, reported using ENTREQ and PRISMA. Quality assessment was performed for the individual studies. Thematic analysis and the GRADE-CERQual approach were used in the synthesis including the assessment of confidence in the evidence. RESULTS: The model was tested against the evidence in 15 studies appraised with moderate or high quality and found represented in the literature but needed to be expanded. A refined model with a moderate/high confidence level of evidence presents elements to be used in a holistic care process; The nurse is guided to establish a trusting relationship with the patient and enable communication to identify and meet pain management needs using pharmacological and non-pharmacological management. Nurse leaders are guided to support this process by providing the right contextual conditions. CONCLUSIONS: The strengths of the confidence level in the refined model, and that it is represented from the nurse and patient perspectives in nursing research across countries and cultures, support our recommendation for empirical evaluation. IMPLICATIONS FOR THE PROFESSION AND/OR PATIENT CARE: The model links the knowledge of pain management elements from individual studies together into actions to be performed in clinical practice. It also outlines the organizational support needed to make this happen. Nurses and nursing leaders are suggested to test the model to implement person-centred pain management in clinical practice. PATIENT OR PUBLIC CONTRIBUTION: No patient or public contribution. IMPACT: What Problem Did the Study Address? There is a need to transfer available evidence of person-centred pain management into practice to relieve the patient from pain. What Were the Main Findings? Person-centred pain management is of high priority for patients and nurses around the world and can be performed in a holistic care process including patient-nurse trust and communication, supported by contextual conditions to deliver timely pharmacological and non-pharmacological pain management addressing the patient's physical, psychosocial and relational care needs. Where and on Whom will the Research Have an Impact? The model is to be tested and evaluated in clinical practice to guide the providers to relieve the patient from pain. REPORTING METHOD: Relevant EQUATOR guidelines were used to report the study: The Preferred Reporting Items for Systematic Reviews and Meta-Analyses Statement.

ABSTRACT

INTRODUCTION: Understanding a patient's culture is imperative to providing holistic patient care. The purpose of the study is to describe and explore the lived experiences of non-Muslim hospital-employed registered nurses providing care for Muslim patients in the United States. METHOD: This study used a qualitative exploratory research design based on semi-structured interviews utilizing Husserlian phenomenology. A snowball technique was used to recruit the participants. RESULTS: Ten nurses who cared for hospitalized Muslim patients were interviewed and three major themes emerged from the participants' narratives: Nurse-Patient Relationship, Nurses' Knowledge and Western Health Care Systems, and Family Influence. DISCUSSION: Muslim patients have cultural expectations and differences that may not be anticipated by nurses, which affects nurses' experiences when providing care. As the Muslim population continues to grow in the United States, there is a need for increased education on culturally congruent care to assure the highest quality of nursing care.

ABSTRACT

ABSTRACT: Nurse-provided spiritual care includes support of patient spiritual practices such as prayer. However, limited evidence exists about how nurses respond when a patient requests prayer. A subsample of nurses (n = 381) from a larger study responded to two open-ended questions in an online survey in response to a prayer scenario. Among these mostly Christian nurses, 97% indicated willingness to pray. Content analysis revealed a five-component structure for praying: Open, Set the Stage, Request, Wrap-up, Close. The structure provides a template for future research and nurse prayer in clinical contexts.

ABSTRACT

Humans are continuously storying and re-storying themselves through language and socially organizing language into narratives to create meaning through experiences. Storytelling through narrative inquiry can bridge world experiences and co-create new moments in time that honor human patterns as wholeness and illuminate the potential for evolving consciousness. This article aims to introduce narrative inquiry methodology as a caring and relational research approach aligned with the worldview grounding Unitary Caring Science. This article uses nursing as an exemplar to inform other human science disciplines interested in using narrative inquiry in research, while defining essential components of narrative inquiry through the theoretical lens of Unitary Caring Science. By exploring research questions through this renewed understanding of narrative inquiry informed by the ontological and ethical tenets of Unitary Caring Science, healthcare disciplines will be knowledgeable and prepared to facilitate knowledge development to contribute to the sustainment of humanity and healthcare beyond eliminating the cause of illness and into the experience of living well with illness. The co-creative process of narrative inquiry as a caring and healing inquiry can guide collective wisdom, moral force, and emancipatory actions by seeing and valuing human experiences through an evolved holistic and humanizing lens.

ABSTRACT

AIM: To illuminate the meaning of newly graduated registered nurses' experiences of caring for patients in emergency departments during the COVID-19 pandemic. DESIGN: A phenomenological hermeneutical study guided by Lindseth and Norberg. METHODS: In-depth one-on-one interviews with 14 nurses from five hospitals were conducted from March to November 2020 and analysed using thematic analysis. The consolidated criteria for reporting qualitative research (COREQ) were used as the reporting guideline. RESULTS: The findings comprise one main theme Caring through barriers and three themes with sub-themes. In the first theme, having intention to care, participants revealed their dedication to care for patients during the pandemic despite extensive stress, little experience and skills. The second theme, with tied hands in human suffering, illuminates experiences of being disconnected from the patient, overwhelmed by responsibility and unable to relieve suffering. The third theme, feeling inadequate, reveals experiences of lack of support and doubts meaning less space to develop into the nurse one wants to be. CONCLUSION: Findings reveal a new understanding of new nurses' experiences during times of crisis. The essence of caring in the emergency department during the pandemic can be explained as mediated through spatial, temporal and emotional barriers preventing new nurses from providing holistic care. IMPACT: The results may be used as anticipatory guidance for new nurses and inform targeted support interventions to support new nurses entering the profession in crisis conditions. PUBLIC CONTRIBUTION: This study involved new nurses in semi-structured interviews.

ABSTRACT

Dengue, a viral infection transmitted by Aedes aegypti, can evolve into severe clinical pictures. In patients with warning signs, constant monitoring of vital signs is necessary, requiring hospitalization. The nursing staff has been considered key in the approach algorithms for these patients. Therefore, good care performance and a harmonious relationship with the patient may be essential. The humanization of care is imperative to help safeguard human dignity in harmony with bioethical principles. Therefore, a systematic review of the literature was carried out with the purpose of identifying the state of the art on humanized nursing care and its relationship with the satisfaction of the hospitalized patient. 50 articles related to the research variables were identified. The results point to the scale in the measurement of transpersonal human care based on Watson's theory, as the most frequent; likewise, the patient satisfaction questionnaire with the quality of nursing care, as the most used method. Regarding the elements of nursing care, they are grouped into two dimensions; humanized care and praxis. Finally, patients associate quality of service with user satisfaction, positively perceiving the humanization of care. It is recommended to implement strategies and identify critical points of care, improvement plans and training to sensitize nursing staff towards good patient treatment, from a holistic and comprehensive approach(AU)

ABSTRACT

ABSTRACT: Educating prelicensure nurses about the positive outcomes of prayer can improve the potential for nurses to develop their skill and readiness to pray with or for patients as is appropriate and ethical. When nursing students learn to pray with their patients and families, the trusting component of the nurse-patient relationship becomes strengthened. As faculty and preceptors role-model praying with patients, students can begin to develop competency in providing spiritual care.

ABSTRACT

Objective: This study aimed to investigate the effect of building Trust in Nurses (TN) on improving respiratory function, quality of life (QoL) and the self-management ability of patients with bronchopneumonia. Methods: A total of 92 patients hospitalized in The Second Affiliated Hospital of Harbin Medical University in China between November 2019 and October 2020 were prospectively included in the study. Patients were randomly assigned to either the TN group (intervention group; n = 46) or routine nursing (control group; n = 46). Clinical symptom improvement time, pre- and post- pulmonary function (PF) after the nursing intervention, QoL, self-management ability, patient compliance and satisfaction in the 2 groups were recorded and compared. Results: Clinical symptom improvement time, including the resolution of cough, lung rales, expectoration and wheezing in the TN group were significantly shorter than in the control group (P < .001). PF, including forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1) and mid-maximum expiratory flow (MMEF) velocity 25% to 75% in the TN group were significantly better than in the control group (P < .001). In addition, patient QoL based on the 36-Item Short Form Health Survey (SF-36) (P < .001), self-management ability based on the Exercise of Self-Care Agency Scale (ESCA) (P < .001), good compliance rate (P = .024) and satisfaction rate (P = .024) in the TN group were all significantly better than in the control group. Conclusion: Building TN was an effective intervention in patients with bronchopneumonia, and was beneficial for improving clinical symptoms, PF, QoL, self-management ability, compliance and the satisfaction rate in patients.

ABSTRACT

Nurse theorists have addressed the primacy of the phenomenon of caring, aiming at providing a framework that captures the complex nature of caring. Several theorists emphasized the mechanical facet of care while others emphasized the holistic aspect of care. Spirituality as a central concept in caring theories was targeted in this manuscript; as it a fundamental aspect of holistic care. Watson's Human Caring Science Theory and Lovering's Crescent of Care Nursing Model represent two distinctive approaches to caring in the dimension of spirituality. A compare/contrast approach is used to depict the similarities and differences between both works, focusing on spirituality as a common concept. The outcome of the comparison showed that spirituality is a multidimensional metaphysical concept that both theorists had identified as an indispensable core aspect of holistic nursing and is context-bound in terms of its dimensions, applications, and meaningfulness.

ABSTRACT

Objective To relate the aspects of religiosity / spirituality in nurses' practice. Method A cross-sectional study was carried out with 31 nurses from a teaching hospital in the interior of SP, who answered a structured questionnaire of the Portuguese version of the Duke-DUREL Religiosity Scale modified by Moreira-Almeida et al. Results Nurses positively recognize the influence of religiosity / spirituality in the health-disease process, portraying that they have already questioned patients about the subject, although they felt uncomfortable. Lack of time, fear of imposing their point of view and not feeling prepared, are reported as difficulties. The nurses mention that they acquired knowledge about religiosity / spirituality in their own religion, since the topic was not addressed in undergraduate studies or in continuing education at the hospital. Thus, they suggest that religiosity / spirituality R/E may become mandatory subjects, due to its importance. Professional experience directly contributes to changes in religious beliefs or behaviors in 10 (32%) of nurses, attributed to living with patients' pain and suffering, impacting the way of thinking and valuing life and health. Conclusions There is a potential to be explored by the continuing education of nurses in relation to the approach to religiosity / spirituality, aspects that will allow comprehensive care and that will have repercussions on the facing of difficult situations of suffering by patients, families and professionals.

ABSTRACT

ABSTRACT Nursing as a profession, with its work processes directly affected by the pandemic, finds qualified and guided listening in communication in light technologies as strategies to improve the interpersonal relationship established with the health team, the family and the patient (Rodríguez, Belarmino, Custódio, & Ferreira Júnior, 2020). Although this communication had to be modified in the face of the pandemic, it continues to be essential for nursing care. Objective: To describe the benefits of interpersonal relationships in nursing care. Methods: integrative review, January 2020- July 2021, and starts with the research question, What benefits does the interpersonal relationship have in the satisfaction of nursing care in times of pandemic? The line of research: Nursing Practice. The search strategies used were the MEDLINE databases with its PUBMED search engine, EBSCO Host, Lilacs, Redalyc and EMBASE; in English and Spanish. The information search was done by restricted specificity to MESH and DECS terms and to broaden the search, combinations and Boolean engines were performed. Conclusions: The communication process is not an easy task and requires a lot of effort among those involved. The communication was satisfactory for the patient and the family when it aims at the quality of health and the objectives of patient safety. There is interpersonal communication of dissatisfaction when insufficient updates are given on the patient's condition and when the family is not consulted on the decision-making of their relative.
